    The LPN provides direct nursing care to residents and supervises the nursing tasks and activities performed by certified nurse aides and other nursing department non-licensed staff. Such supervision must be in accordance with current federal and state regulations and laws. The ideal candidate is licensed as a Licensed Practical Nurse in the State of PA and has a working knowledge of pharmacology and good leadership skills. Experience working with an elderly population is preferred.

     

    Hours: Part Time - 4 AM - 8 AM

     

     
     

    Responsibilities

    • Completes resident data collection and nursing assessments within scope of practice
    • Identifies and documents resident problems, symptoms, and changes in condition and takes appropriate action
    • Participates in the development of an interdisciplinary plan of care
    • Implements nursing care, including administering medications and performing medical treatments and nursing procedures within scope of practice
    • Evaluates the effectiveness of the plan of care and makes changes when necessary within scope of practice
    • Takes report from out-going nurse and gives report to in-coming nurse
    • Provides leadership to nursing personnel assigned to unit/shift
    • Monitors the implementation of the plan of care for assigned residents and supervises the tasks and activities of the certified nursing assistants and nursing non-licensed staff
    • Performs narcotic count at beginning and end of each shift as directed by policy
    • Communicates effectively with others (residents, families, visitors, and co-workers)
    • Carries out facility policies and procedures
    • Documents information in an informative and descriptive manner that reflects the care provided to the resident and the resident’s response to the care
    • Understands and follows all HIPAA guidelines
    • Understands and follows infection control practices
    • Reports and investigates all allegations of resident abuse, neglect, and misappropriation of resident property
    • Ensures confidentiality of protected health information in accordance with established policies and procedures
    • Ensures that resident confidentiality, privacy, and dignity are respected in accordance with residents' rights
    • Develops and maintains effective communication with inter-departmental personnel
    • Reports occupational exposures to blood, other potentially infectious materials, and hazardous chemicals in accordance with established policies and procedures
    • Reports, investigates, and takes corrective action for discrepancies concerning physician’s orders, medication/nursing treatment errors, documentation concerns, etc., and reports to immediate supervisor
    • Completes all appropriate documentation with regard to resident, employee, and visitor accidents/incidents, notifies all appropriate individuals, and takes appropriate corrective action
    • Follows all personnel policies as outlined in Personnel Policy Manual and Nursing Services section of Policy and Procedure Manual
    • Attends mandatory in-service education programs and continuing professional education
    • Completes Quality Assurance audits as assigned
    • Performs other duties as assigned in accordance with policies/procedures, rules, regulations, and guidelines

     

    Qualifications

    • Licensed as a Licensed Practical Nurse in the State of PA
    • Able to read, write, speak, and understand the English language
    • Working knowledge of pharmacology
    • Competent leadership skills
    • Adequate problem-solving skills
    • Positive interpersonal skills in working with residents, families, and interdisciplinary team members
    • Awareness of environmental safety
    • Able to plan, develop, implement, and interpret the programs, policies, and procedures for quality care
    • Adequate organizational and interpersonal skills
    • Willing to be subject to call-back during emergency conditions, i.e. severe weather, evacuation, disaster, etc.
    • Must be physically able to perform the essential functions of the job
